Cook was born in Brooklyn, New York, on May 4, 1940. He spent his childhood in Woodside, Queens and later moved to Leonia, New Jersey. He majored in chemistry at the Wesleyan University. After that he attended the Columbia University Medical School. He also spent some time in the military service, post which he opened a small practice of medicine.
